Voluntary Participation. I acknowledge that I have voluntarily applied to participate in certain activities made available by the University Union Operation of California State University, Sacramento, Inc. (UUOCI)/The University Union and The WELL (hereafter known as “Union WELL”), at California State University, Sacramento (hereafter known as “Sacramento State”). These activities include, but are not limited to Intramurals, Fitness, Rock Climbing, Open Recreation, Classes, contracted facility service rentals, use of WELL facilities and equipment, Personal Training activities conducted in WELL facilities and elsewhere at Sacramento State under the direction of Union WELL staff (“Union WELL Activities”), and services offered at the Relaxation Station in the University Union. 2. Assumption of Risk. I ACKNOWLEDGE THAT PARTICIPATION IN WELL ACTIVITIES OR ANY ACTIVITIES INCIDENTAL THERETO IS POTENTIALLY HAZARDOUS AND INVOLVES CERTAIN RISKS OF INJURY, INCLUDING, BUT NOT LIMITED TO, LACERATIONS, PULLS AND STRAINS, CONCUSSIONS, BROKEN BONES, LOSS OF LIMB(S), PARALYSIS, OR DEATH. Alumni Membership

To sign up for membership, stop by The WELL Engagement Center or Front Desk with your Alumni Association card or confirmation e-mail. Alumni Membership Process Step 1: Join the Alumni Association The first step to becoming a WELL alumni member is to join the Sacramento State Alumni Association. Since The WELL is not open to the community, we require this membership to re-associate graduates with the Sac State campus through an active affiliation. Alumni can activate their Alumni Association membership online, or in person at the Harper Alumni Association. Once you have joined the Alumni Association, please come to The WELL's Engagement Center or Front Desk to show us your confirmation e-mail or Alumni Association card. Step 2. Select the Best WELL Membership for Your Needs The WELL offers a variety of membership options for Alumni who are current members of the Alumni Association. WELL Alumni Membership options Monthly Recurring Membership: are billed monthly and have a 4-month minimum commitment. Alumni - $35/ mo. Available to Alumni who have an active membership with the Alumni Association Recent Grad - $20/mo. Available to Alumni who have graduated in the past 5 years and are members of the Alumni Association. Pre-paid Alumni - $16/mo. Faculty/Staff Membership

Two Months i. $98.00 Canceling Your Monthly Recurring Membership Affiliate WELL Monthly Recurring members are eligible to cancel their membership after 4 months. Cancellations require a 30-day notice through our online form. Adjustments will not be made after the 26th of the month for the following month. Refunds Refunds are only authorized in the case of a medical necessity (must be supported by a doctor’s note) or a death in the family (documentation required). The method of refund will occur in the same payment type as the original transaction. If cash refund exceeds $50.00 or exceeds the amount of cash tendered in Point of Sale (POS) for the day of refund, then the customer will receive a check by mail. Please submit refund inquiries using the WELL Membership Adjustments form.
